If a player was banned for being in glitched area of a map by mistake and banned, are they forever banned? Do players not get 2nd chances? This player had over 4 thousand hours of normal play and now that he is banned he can never play BF4 again with his friends for one mistake?

He was glitched by a mistake, but he took the advantage of it, he kept abusing the glitch and kept killing people, then he got caught, and now he's asking for a 2nd chance? lol
Doubt he would be banned otherwise...

Depends on the case. Accidental use will be an approved appeal. If it can be shown it was abused, the appeal would be denied.
